SACRAMENT ENROLLMENT FORM

To ensure that your child is included in preparation processes, please complete the Sacrament Enrollment Form and submit it to the Faith Formation Office.

BAPTISM

New and expecting parents are required to attend one baptism preparation session before scheduling their child’s Sacrament of Baptism. All parents bringing a child for baptism are invited to participate in a session.

These sessions not only review the Rite of Baptism, but also engage parents in exploring how they can raise their children in the practice of the Catholic faith.

Sessions are offered every other month.

Celebration of Baptism

Baptisms are usually celebrated during Masses. Contact the Parish Office for the schedule, 616-676-9111.

A Baptism Information Form must be completed to confirm your baby’s baptism.

TRADITIONAL AGE SACRAMENT PREPARATION

Students in second grade prepare for the celebration of the Sacraments of Holy Communion and Reconciliation. Learning takes place in weekly faith formation sessions and with at-home activities.

 

NON-TRADITIONAL AGE SACRAMENT PREPARATION

Older students preparing for the Sacraments of Holy Communion and Reconciliation participate in weekly faith formation sessions at their grade level, participate in sacrament preparation sessions, and receive activities to do at-home with their families.

Holy Communion & Reconciliation Dates to Remember:

First Reconciliation Parent-Child Retreat:           1-3 p.m. Sunday, October 23, 2016

Dates for First Reconciliation:                             4:30-7:30 p.m.

January 3, 5, 10, 12, 17, 19, 24, 2017

First Communion Parent-Child Retreat:              1-3 p.m. Sunday, March 5, 2017

Dates for First Holy Communion:                       2 p.m. Sundays, April 23 or 30, 2017

MARRIAGE

Participation in a Pre-Cana Conference is required for engaged couples. Pre-Cana Conferences offer engaged couples preparation for their marriages by lead couples sharing their experiences of married life on a variety of topics. Engaged couples are then provided the opportunity to have those “must-have” conversations.

 

 Pre-Cana Conferences:                   Friday, Oct 14, 7-9 p.m.; Saturday, Oct 15, 9 a.m.-4:30 p.m.
